Laparoscopic Management of Interstitial Pregnancy

Abstract
Interstitial or cornual pregnancy is a rare occurrence, accounting for approximately 2 to 6 percent of ectopic pregnancies and is associated with an increased risk of severe hemorrhage. Conventional management of interstitial pregnancy has been
salpingectomy and cornual resection or hysterectomy by laparotomy because of a belief that laparotomy was almost inevitable and laparoscopic treatment rarely possible. However, as with other site of tubal pregnancies, more sensitive assay for
¥â-HCG and
improved techniques of ultrasonographic evaluation of early pregnacies enable earlier detection of an unruptured ectopic gestation.
We experienced 304 cases of ectopic pregnancies from September 1989 to November 1995. Among these, we experienced 19 cases of interstitial pregnancies.
We present the results of laparoscopic management of conservative or non-conservative interstitial pregnancy and review the literature concerning the interstitial pregnancy.
@ES The results were as follows.
@EN 1. The total number of ectopic pregnancies were 304 cases and among these interstitial pregnancies is 19 cases. The incidence was 6.3%.
2. The mean age of patients in 31.8(¡¾5.1) years and mean parity was 1.3(¡¾0.7)
3. The number of cases of which previous pelivc surgery has been done were 5(26.3%). These are salpingectomies, fimbryoplasties, cesarean sections.
4. The number of cases experiencing previous ectopic pregnancy were 4(21.1%).
5. Mean gestational age at the time of operation was 63.7(¡¾11.4) days.
6. Among 19 cases, we conducted salpingectomy and cornual resection in 15 cases and salpingotomy in 3 cases by laparoscopy. One cases of salpingectomy and cornual resection was by laparotomy.
7. Among 19 cases, 7 cases were ruptured state and 12 cases were unruptured state at time of operation.
8. There were no specific complications.
